It is the policy of Kreinbrook Psychological Services (KPS) that all staff preserves the integrity and the confidentiality of Protected Healthcare Information pertaining to our patients. The purpose of this policy is to ensure that our practice and its professional staff have the necessary medical and Protected Healthcare Information to provide the highest quality mental healthcare possible, and insure the rights of individuals to protect their Healthcare Information. KPS has the right to release or obtain patient records with other treatment professions for the purpose of treatment, without the patient’s consent. Other than for treatment purposes all Protected Healthcare Information is confidential and can not be released without the patient’s written consent.
You have the right to be given a copy of your Psychological/Psychiatric treatment records. That would include: a Clinical Interview, Psychiatric Evaluation, and Medication Review Notes. Kreinbrook Psychological Services does not provide psychotherapy notes to individuals requesting their records.
You have the right to look at and review your Psychological/Psychiatric records. We require a written request to review records. An appointment time will be scheduled for this review.
Individuals that have reviewed their records have the right to request corrections of inaccurate or incomplete private Healthcare Information in their Psychological/Psychiatric records in accordance with the law and professional standards. KPS reserves the right to deny this request under certain circumstances.
You have the right to receive a list of instances in which KPS has disclosed your Healthcare Information for purposes other than treatment or payment over the last five years.
You have the right to request that we place additional restrictions on our use or disclosure of your health information. KPS is not required to agree to these additional restrictions but if we do, we will abide by our agreement (except in an emergency). You have the right to have your Psychological/Psychiatric records sent by regular mail, or e-mail written consent and location; the mail address or e-mail address must be provided. You will be informed of the fee for providing your records.
Your Healthcare Information can only be provided to your Primary Care Physician or other healthcare providers with your knowledge and written consent.
Kreinbrook Psychological Services maintains a non-discrimination policy in all aspects of our practice. No individual will be discriminated against due to race, religion, ethnicity, cultural background, age, persons with disabilities, gender or sexual orientation, or any other characteristic protected under applicable federal or state law.
All professionals and staff of our practice must adhere to this policy. Our practice will not tolerate violations of this policy. Violations of this policy are grounds for disciplinary action, up to and including termination of employment and criminal or professional sanctions in accordance with our practice’s personnel rules and regulations.
